  To order by mail, download and print these documents in .pdf format.

  Fill out all the pertinent information required, sign and date forms. Pack the completed forms into a well protected envelope or package along with:

  • Your project master disc
  • Graphics layout discs, original art or instruction for design
  • Printouts of your graphics files
  • Any special instructions that will help us expedite your project
  • A check for 60% of the order total as a deposit or credit card info

  Next Day Disc recommends that you send your package in a manner in which you can track it's progress to us: i.e UPS, Fed-ex, or US post w/tracking. Please insure your package for it's full value. Next Day Disc cannot accept liability for packages damaged in transit. Your Next Day Disc representative will contact you once we receive your package to ensure that we fully understand your requirements and that we have everything we need to do fill your order.
